The Political Reform Act requires elected officials to disclose payments made at their behest, principally for legislative, governmental, or charitable purposes (not for personal or campaign purposes) on Form 803. Forms for the past calendar year are provided below:

A recipient committee is any individual (including an officeholder or a candidate), group of individuals, organization, or any other entity that receives contributions totaling $1,000 or more during a calendar year. Under the Political Reform Act, a recipient committee to file a statement of organization using Form 410 once it has received $1,000 in contributions.
